ICU Patient Progress Summary (23.07 – 30.07)

Between July 23 and July 30, the patient remained in critical condition, requiring ongoing sedation, mechanical ventilation, and inotrope support. The patient consistently presented with Atrial Fibrillation (AF) managed by Cordarone, required intermittent Continuous Veno-Venous Hemodiafiltration (CCVHD) for renal support, and struggled with weaning attempts from ventilation via CPAP trials.

Key clinical challenges included persistent anemia (requiring multiple blood transfusions), elevated CRP levels peaking at 260, and deranged liver function tests. Wound cultures revealed the presence of Klebsiella and Candida. Additionally, the patient experienced fluctuating blood pressure, unstable blood glucose levels, and varying renal function (eGfr levels ranging from 18 to 59).

Treatment involved a complex array of medications including Noradrenaline and Vasopressin for blood pressure regulation, various antibiotics, and TPN/NG feeds for nutrition. Physiotherapy was performed twice daily. By July 30, the patient remained intubated after failing CPAP trials, while CCVHD was temporarily suspended.
