The video opens in a lab where Dr. Blaine Rothschild is testing 'Diet Phantasma' soda on subjects. The first subject, number 37, experiences agonizing pain, bleeds from his face, and is forced to drink the soda by tentacles from the can, leading to his death and facial destruction. Cleaners quickly dispose of the body, indicating this is a recurring incident.
Best friends Lacie and Kaleigh celebrate their last Halloween before college. They discover a brightly lit house and enter despite Kaleigh's fear. Inside, they find spilled milk, a still-breathing fetus, and a strange woman referred to as 'Mommy.' They also discover a large cage, enormous footprints, a breast pump designed for six breasts, and a woman connected to a milk dispenser. They realize the house is a trap when doors vanish and windows are boarded. They encounter a cheerleade, and flee from a six-breasted 'Mommy' who captures and transforms Lacie and Kaleigh into her 'babies' with swollen cheeks.
Back at Dr. Blaine’s lab, subjects 38 and 39 also die gruesomely after trying the 'Diet Phantasma.' The scene then shifts to Spain, where Enric, the sole survivor of a Halloween party massacre, is interviewed by police. All victims had crushed bones and missing eyes. Enric is taken back to the crime scene and reenacts entering a medium's house through an underground tunnel with his friends. There, they find a room with eerie drawings and a rotary telephone.
In the present, Enric is terrified by the reappearance of the rotary phone. Flashbacks show the group discovering the Latin phrase “ut supra sic infra” (As above, so below). Enric repeating the phrase three times causes the phone to ring and him to shake violently. The police make Enric repeat the phrase again, and he becomes possessed, vomiting his friends' eyes and tearing out the officers' eyeballs, revealing how the bones were crushed.
Subject 40 survives the 'Diet Phantasma' test but becomes deranged and vomits a black substance. She is burned alive, but Dr. Blaine is hopeful. Next, a group of friends, Haley, Austin, Lauren, and Josh, go trick-or-treating. They encounter a bowl of bizarre candies. Austin eats one and is then swallowed by the bowl, followed by Josh and the girls, and wake up in a dark cellar. Here, they are hunted by an entity called 'Fun Size' and are processed into candies, losing body parts like brains, tongues, eyes, and genitals. Austin's body is turned into candy, and the group finds the key hidden inside one of the packaged candies. Hailey is blasted apart by gumballs and willingly sacrifices herself to the conveyor belt. Josh is sucked into the machine, and Lauren escapes through a vent, only to be attacked by Fun Size outside.
Tim Kaplan, who runs 'Kidprints' video documentaries to track children, is warned by his wife about rising kidnappings on Halloween. An officer informs Tim that a teen, Olivia, has disappeared. Tim goes to retrieve Olivia's tapes from his office, where he discovers footage of tortured and kidnapped children. He finds Olivia being recorded by his tech guy, Bruce. Bruce captures Tim, places a young boy's skin on his face, and then brings Olivia into a basement. Olivia and another teen, misunderstanding Tim's plight, stab him to death, thinking he is an accomplice. Bruce then kills the teens. Tim is framed as the murderer in the news, while Bruce takes over 'Kidprints,' pretending to be a hero ensuring children's safety.
In Dr. Blaine's lab, five new subjects are tested. Subject 47, a little girl, calmly drinks the soda, her eyes turn white, and she speaks in a possessed voice, naming 'Diet Phantasma' as her favorite. Dr. Blaine views this as a breakthrough, regardless of the chaos and destruction caused by the other subjects. He orders the product to be packaged and shipped. The story then transitions to Keith, Nancy, and their son Zac, who annually create 'Dr. Mortis’ House of Horrors.' Teenager Zac no longer wants to participate but is convinced by his mother.
Keith and Zac visit an antique store for props. Keith steals an old vinyl record called 'Halloween Horrors,' which warns that playing it on Halloween will unleash riders of hell. Nancy plays the record during the grand opening of their haunted house. Objects begin flying, and the record catches fire. In the cemetery section, an old man complains about the fake ghosts, but then real ghosts in white sheets appear. The old man is brutally killed, and panic ensues as more ghosts relentlessly chase and kill visitors. Fake dead bodies come to life, killing more children. The family and a little girl reach a castle, then a torture chamber where the girl is killed, and an old woman is killed by an executioner before Nancy kills him with an axe. The surviving family reaches the final room where the little girl is devoured by a witch. Nancy kills the witch. As they leave, another witch snatches Zac’s camera and wreaks havoc, killing trick-or-treating children. The video concludes with a ‘Diet Phantasma’ commercial featuring Subject 47, advertising the soda as being made with real ghosts and poltergeist extract, as the girl drinks it and speaks in a possessed voice.
