Young Ziggy Berman is attacked by Sheila at Camp Nightwing. Counselors break up the fight, and Ziggy is told to call her mom. Meanwhile, Ziggy's sister Cindy works as a counselor with her boyfriend Tommy, dealing with Alice and Arnie doing drugs.
Ziggy visits Nurse Mary Lane, who hints at the justification of some murders. Cindy and Ziggy argue about leaving Shadyside. Mary attacks Tommy, who knocks her out. Cindy, Tommy, Alice, and Arnie find Mary's diary revealing Sarah Fier's legend and a map to her old home. They discover Tommy's name carved on a wall in the caverns, and he attacks Arnie with an axe.
Nick helps Ziggy prank Sheila. Jeremy is brutally murdered by Tommy. Ziggy and Nick share a moment before discovering Jeremy's body. Tommy continues his killing spree, targeting counselors and campers.
Cindy and Alice navigate the caverns and find a beating organ, which Alice touches, seeing visions of past killers and future victims. Alice injures her leg, leading to a heartfelt confession between the two about their past.
Ziggy finds Sheila and knocks her out. Cindy and Alice find an exit from the caverns. Tommy decapitates Gary and slashes Nick. Ziggy misses the bus to safety. Cindy and Alice find Ziggy in the mess hall, where Cindy ultimately kills Tommy. Alice finds Sarah's skeletal hand. Ziggy's blood drips on the hand, reanimating Tommy, who then kills Alice. Cindy beheads Tommy.
Ziggy and Cindy reach the Hanging Tree but are confronted by Tommy and other killers. Cindy sacrifices herself to allow Ziggy to try to place Sarah's hand. Both sisters are attacked and die. Nick revives Ziggy with CPR, but Cindy is gone.
Berman reveals her true name is Christine (Ziggy). Nick refused to acknowledge the curse to protect his career. Deena and Josh ask about Sarah's hand, which is still under the Hanging Tree (now the Shadyside Mall). Deena takes the hand to Sarah's grave and is transported to 1666, becoming Sarah.
