Clinical Assessment and Care Planning

Therapists are responsible for conducting initial OASIS assessments within 48 hours of referral and performing re-evaluations every 30 days. They must develop and revise personalized plans of care in consultation with physicians and other care-team members to ensure regulatory compliance and meet patient goals.

Documentation and Communication

The role requires meticulous preparation of clinical and progress notes, summaries, and updates regarding patient status. Effective communication is essential for coordinating care with families, agency staff, and physicians, as well as reporting changes in condition to the Director of Nursing or Administrator.

Supervision and Operational Tasks

Therapists oversee Therapy Assistants and Home Health Aides, which includes scheduling supervisory visits and co-signing documentation. Additional responsibilities include staff in-service training, participation in quality assurance and peer reviews, equipment management, and assisting patients and families with safe home therapy programs.
